Please fill out one Parish Investment & Affiliation Form per family. This form must be signed by your parish priest or business manager and then returned to the school. Parish rate is not guaranteed until this signed form is submitted. The parish is asked to help pay a subsidy in order for you to receive the discounted parishioner rate. In doing so, each parish asks for your support through its own policies and requirements.
